Youth Intake Process
Once a referral has been accepted, the following process will be initiated:
- A written, or verbal, referral to be made to the Senior Youth Treatment Worker or Unit Coordinator.
- Written approval for funding of the per diem rate must be received in writing prior to placement for referrals attached to a child care agency.
- Admission Application and Medical Summary, as well as all consent forms are forwarded by referral source. The referral sources are also requested to send any other information such as psychological assessments, and discharge reports from previous placements, as might be pertinent to plan effectively for treatment.
- If the referral resides in Winnipeg, an assessment interview is conducted prior to placement. An initial interview allows staff to introduce the program to the prospective resident, significant others and/or appointed guardian and to assess the seriousness of the referral to placement.
- When all necessary documentation is complete, and depending on the availability of bed-space, the youth is placed into the program.
The youth is provided an orientation to the facility. Immediate needs such as clothing, room, toiletries, etc. are looked after. The youth is then introduced to staff and residents and given the Youth Services Handbook . This handbook outlines the rights, expectations/rules, and grievance procedure.
